PHYSICAL THERAPY SERVICES

M & M Home Care’s Physical Therapists (PTs) are movement experts specializing in evidence-based treatment approaches collaborating with clients to improve their quality of life through prescribed exercise, hands-on care, and patient education. Our PTs are skilled in evaluating each client in order to develop a uniquely tailored treatment plan to improve their ability to move, reduce or manage pain, restore function, and prevent disability.

By combining manual therapy, therapeutic exercise, neuromuscular re-education, and functional activities our physical therapists are able to provide an outpatient physical therapy experience in your home. By addressing a variety of conditions within your home, our practitioners are able to individually tailor your therapy to help you move best in your own environment. M&M Home Care’s physical therapy plan of care may include the following:

  • Manual Therapy including, but not limited to;
    • Joint Mobilization and Manipulation
      • Improve pain and swelling
      • Improve joint mobility to assist with improving range of motion and function
      • Improve ability to achieve proper positioning
    • Manual Muscle Stretching
      • Improve muscle length and play that can lead to decreased range of motion
      • Improve pain levels
      • Improve ability to achieve proper positioning
    • Soft Tissue Mobilization, Functional Massage, and Scar Tissue Mobilization
      • Improve pain levels, swelling, and tolerance to passive and active range of motion
      • Improve muscle length and play that can lead to decreased range of motion
      • Improve quality of scar tissue mobility and healing
      • Improve ability to achieve proper positioning
    • Therapeutic Exercise
      • Targeted strength training to assist with improving function
      • Improves cardiovascular and muscular endurance
    • Neuromuscular Re-education
      • Improves coordination, movement patterns, balance, posture, and proprioception
    • Therapeutic Activity
      • Specific activity training including bending, lifting, reaching, transfers, walking, and recreational activities to improve functional performance
    • Wheelchair Seating Evaluation & Fitting
      • Assessing postural needs to determine the most appropriate support
      • Determine the most appropriate means of mobility to allow independence and/or ease client care for caregiver
