Our highly skilled dietitian can work with you to set up appropriate nutrition systems, provide education, and work as part of your team to provide medical nutrition therapy in the home. We can help with diabetic and special diet counseling, tube feedings, home IV use, ostomy and wound healing.
Our primary goal is to improve or maintain the nutrition and hydration status of individuals in the home health setting by supporting the patient and the family with education and training based on common nutrition goals of the patient and physician. Nutrition assessment, diet instruction and materials can be provided to patients and/or caregivers.
